RESIDENT HEALTH & PERSONAL CARE

The Director of Care - Long-Term Care is frequently the first point of contact with potential future residents of Golden Birches Terrace, as they manage the waiting list. This provides an early opportunity to become familiar with each resident, assess individual needs, and contribute to the care team's efforts to assist residents in their Activities of Daily Living.

The Director of Care-Long-Term Care co-ordinates regular care team meetings to review each resident's care requirements and formulate their treatment plan, and is available to meet with residents upon request, as well as to provide support and help in accessing the appropriate resources.

Nursing and Personal Support Worker care is available to every resident 24-hours a day to assist with Activities of Daily Living (i.e. bathing, grooming). Registered staff provides assessments, medication administration and prescribed treatments.

A resident’s personal physician provides ongoing medical care. Further, Golden Birches has a Provider on call on a 24-hour basis. If necessary, hospitalization arrangements will be made in either the Acute Care Unit at our Blind River site, or another facility.

CAREGIVER ID PROGRAM

In Ontario, there are ~4 million caregivers who provide physical or emotional support to a family member, partner, friend or neighbor.

“Caregiver ID” is a program created by the Ontario Caregiver Organization (OCO) that recognizes caregivers as essential partners in a patient’s, resident’s or client’s care. This program has been adapted at NSHN and seeks to identify, prioritize, and equip Essential Caregivers with the tools and resources required to support and care for themselves as caregivers while they continue to provide care for and support their loved ones.

Residents at Golden Birches are empowered and supported to choose and identify their preferred Essential Caregiver. At NSHN, Essential Caregivers:

  • May be a relative, friend, or significant other and may not necessarily live with the patient/resident/client. They may also be an appointed guardian, a cultural or community member or a translator.

  • Can provide support and care for short periods of time (e.g. days) or for extended periods of time.

  • Is not a casual visitor, but rather someone who comes to the facility to assist with the daily changing care and needs of the patient.

  • May be allowed access to the facility in periods of heightened restriction, when casual visitors may be prohibited.
